Insurance
I am a private pay practitioner. However, many insurance carriers cover out-of-network providers. I provide all of the necessary forms for you to submit to your carrier. If you plan on submitting to your insurance provider, I would suggest you first call them to find out if services are covered.

Reduced Fee
Reduced fee services are available on a limited basis.

Payment
Cash, check and all major credit cards accepted for payment.

Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the treatment as booked.
